How to Change an Advance Directive

As a registrant, you may suspend, revoke, replace, edit or delete your advance directive.

To Make Changes to Your Advance Directive

1. You must notify the registry in writing as soon as possible of any changes to your advance directive, i.e. emergency contact, or personal information submitted with your registration agreement.

2. Mail or fax a newly executed Advance Directive form or instructions along with an Authorization to Change form to:

How to Revoke the Appointment of an Agent, Suspend, or Delete your Advance Directive

1. You must provide a written statement or an Authorization to Change form that that has been properly witnessed.

2. Mail or fax an Authorization to Change form to:
